Quality of Life and Endoscopic Outcomes Following Functional Endoscopic Sinus Surgery in Patients with Chronic Rhinosinusitis: A Prospective Cohort Study at Fergana Regional Hospital

Background: Functional endoscopic sinus surgery (FESS) is the primary surgical intervention for medically refractory chronic rhinosinusitis (CRS), yet comprehensive prospective outcome data from Central Asian populations remain limited. Objective: To evaluate quality-of-life and endoscopic outcomes following FESS in patients with CRS, comparing phenotypes with and without nasal polyposis. Methods: We conducted a prospective cohort study of 56 consecutive adult patients undergoing FESS at Fergana Regional Hospital between January and August 2025. Primary outcomes included change in 22-item Sinonasal Outcome Test (SNOT-22) scores and Lund-Kennedy endoscopic scores from baseline to 6 months postoperatively. Statistical analyses included paired t-tests, independent t-tests, and effect size calculations. Results: Mean age was 35.7±10.7 years; 48.2% had CRS with nasal polyps (CRSwNP). SNOT-22 scores improved significantly from 66.8±10.8 preoperatively to 24.0±9.6 at 6 months (mean change 42.8±10.7, t=29.9, p<0.001, Cohen's d=4.18). Lund-Kennedy scores decreased from 8.5±1.9 to 2.8±0.8 (mean change 5.6±1.3, t=31.9, p<0.001). All patients achieved clinically significant improvement (≥10-point SNOT-22 reduction). No significant difference in SNOT-22 change was observed between CRSwNP and CRS without polyps (CRSsNP) groups (41.0±9.8 vs 44.5±11.4, p=0.235). Revision surgery rate was 7.1%. Conclusion: FESS yielded substantial and durable improvements in quality of life and objective endoscopic findings across both CRS phenotypes, supporting its efficacy in the regional population.
