Enhanced Recovery After Major Gynecologic Surgery: A Prospective Cohort Study of Postoperative Outcomes

Background: Enhanced recovery after surgery (ERAS) protocols aim to reduce perioperative stress and accelerate recovery, but their impact in major gynecologic surgery remains heterogeneous. Methods: In a prospective cohort, women undergoing major gynecologic surgery were managed either with routine perioperative care or an ERAS protocol including preoperative counseling, minimal fasting, multimodal analgesia, early mobilization, and early oral intake; postoperative complications within 30 days and length of stay were compared between groups. Results: The ERAS group showed a lower overall complication rate and shorter postoperative hospital stay than the standard‑care group. Conclusions: ERAS implementation in major gynecologic surgery was feasible and associated with improved short‑term outcomes, supporting wider adoption and further high‑quality trials.
